If you were referred by DHP, you are in the right place. We’ve created a dedicated intake path to help your practice begin the Solutions 101 assessment process quickly and clearly.
What happens next.
Your Solutions 101 assessment starts in three simple steps. First, you’ll tell us about your practice. Next, you’ll complete the assessment setup. Finally, you’ll review the upload checklist so your team knows what documents will be needed for the next phase.
The process is straightforward, and each page will guide you to the next step.
Complete the intake
Start by telling us about your practice, your role, the high-level issues you want evaluated, and the DHP rep that referred you over.
Complete setup
Once intake is submitted, you’ll move to the setup page to complete the assessment registration and sign the BAA.
Review the checklist
After setup, you’ll see the upload checklist so your team can begin preparing the documents our Data Department will need next.
